Chronic Disease Management Plan (CDM)
The Chronic Disease Management (CDM) Plan is a Medicare initiative that provides rebates for allied health services to individuals with a chronic medical condition. A chronic condition is defined as one that has been, or is expected to be, present for at least six months.
If your condition is being managed by your GP under a CDM plan, you may be eligible to claim Medicare rebates for allied health services, such as speech therapy, occupational therapy, or physiotherapy.
What Does the CDM Cover?
- The CDM allows for up to 5 Medicare-rebated allied health services per calendar year.
- These 5 sessions are shared across all allied health services—not 5 sessions per provider.
(For example, if you see both a speech pathologist and an occupational therapist, the total combined sessions rebated is still 5).
How Do I Access a CDM Plan?
- Speak to your General Practitioner (GP).
- Your GP will assess your eligibility and, if appropriate, create a CDM plan and provide a referral to the relevant allied health professionals.
I’ve Been Referred to May’s Speech Therapy Under a CDM Plan – What’s Next?
- Please provide us with your referral form from your GP.
- You will then be eligible to claim up to 5 rebated sessions within the calendar year, provided your CDM plan has been registered with Medicare.
- Full payment for each therapy session is required.
- Once payment has been received, an invoice will be issued. After payment is confirmed, we will mark the invoice as paid (closed) and return it to you. You can then use this finalised invoice to submit a claim online to Medicare.
How Much Is the Rebate?
- The current Medicare rebate is $60.34 per session (as of 4/05/2025).
